Please fill out the application completely. We understand that it is long and may seem invasive, but please understand the more info we get from you the better fit we can give to you and the type of dog your looking to adopt. We want to ensure the best fit possible for you and your new pet, The adoption fee starts at 200.00 and depends on the dog and possible additional costs we may have had for medical, boarding, special needs etc., additional costs that we may have paid for the dog's needs while in our care. No adoption will be completed that applicant was found to provide false or miss leading information. Zeus's Friends Lost and found pet network reserves the right to deny the adoption to anyone that does not qualify under our adoption guidelines.

If the dog I adopt requires spaying or neutering after adoption due to the age of the dog or other issues I agree that Zeus’s Friends Lost and Found Pet Network, will have the adopted dog or puppy spayed or neutered for me and will make any and all appointments necessary for me. Unless other arrangements are made during the adoption process concerning the spay or neuter of the puppy or dog. I also agree to make sure that the dog or puppy makes it to the appointment when scheduled and pick it up when needed. I understand that Zeus’s Friends Lost and Found Pet Network is only responsible for paying for the spay or neuter and items needed associated to that as well as rabies and pain meds, Any additional things I want done will be done my responsibility, yes no

I further understand that if I do not follow this agreement and act on my own to make appointments for the spay and neuter or other care. I except all responsibility for the procedure and its costs. yes no

By signing this form, I/we acknowledge that all information on this form is true and correct to the best of my knowledge. I/we understand that any misrepresentation of fact may result in Zeus’s Friends Lost and Found Pet Network refusing adoption privileges to me/us. If my/our request for adoption is approved and later Zeus’s Friends Lost and Found Pet Network discovers any of the above information is not true or correct or if I fail to comply with any of the agreements herein this application, Zeus’s Friends Lost and Found Pet Network reserves the right to remove the adopted dog from my home/farm. Upon approval of your application and completion of requirements I/we will receive a certificate of adoption. I understand that upon the finalization of this adoption Zeus's Friends Lost and found Pet Network has no responsibility for any damages incurred by the dog or any veterinary care or treatment needed, unless prior arrangements have been made. I also understand that no refunds will be given for adoption fee's after 1 week of the adoption. yes no
